Mission: Two Rivers Family Health Team is an inter-professional health care team providing excellent primary health care in a respectful, mutually accountable work environment.
Vision: A healthy tomorrow begins today.
The Two Rivers Family Health Team originally began with 12 physicians
coming together in 2006 to provide after hours care to their patients.
Since its inception, the team has grown to 24 physicians working out of 5
separate locations. The three group sites are: Cambridge Healthlink,
Hespeler Medical Clinic, and Forbes Park Medical Centre. The solo sites of
Dr. George and Dr. Snyder round out the team.
As we have grown as a team, we have successfully launched several chronic disease management programs and built a network of allied health professionals to provide added benefits and enhanced care to our patients.
