The purpose of clinical pharmacy services is to collaborate with the entire health care team involved in a patient's “circle of care” to help ensure the best outcomes from medications with the fewest adverse events.

The purpose of the diabetes education program is to provide diabetes education to help patients learn about Pre-Diabetes and Diabetes.  DEP will improve patient and health care provider education surrounding Diabetes and Pre-Diabetes.

An educational program to help children and young people enjoy the best possible health now and into their adult lives. This program will focus on  children’s health in the areas of nutrition and physical activity.

The anticoagulation clinic provides intensive case management for patients with erratic INR measurements, for the purpose of INR stabilization.  Pharmacist/Nurse Practitioner managed anticoagulation can provide improved patient and health care provider education surrounding anticoagulation, both inside and outside the clinic.

Two Rivers Family Health Team has implemented the Baby Friendly Initiative to promote, protect and support breastfeeding in our community.

The Two Rivers Family Health Team Memory Clinic is a specialty clinic dedicated to the early diagnosis and management of patients with memory difficulties.

Mental health services are available within Two Rivers FHT to assist patients with mild to moderate mental health or psychosocial issues.   The mental health team can provide consultation, assessment, brief support and ongoing counseling to help patients enhance coping skills and optimize their overall functioning.

The nursing team within the Two Rivers Family Health Team is a dynamic team of professionals who bring a diverse expertise to patient care.  The team is comprised of 3 Nurse Practitioners, 8 Registered Nurses, and 7 Registered Practical Nurses.  With  experience in community nursing care, home care, outpatient clinics, cardiology and maternal-child care, the team is currently supporting patients in Cambridge Healthlink, Forbes Park Centre and Hespeler Medical Clinic.

Registered Dietitians are trained to counsel and guide individuals with a variety of conditions to optimize their eating plan and nutrition status.   Registered Dietitians can recommend a therapeutic diet to manage certain disease conditions, or provide education about healthy eating and lifestyle changes to enhance patients quality of life and prevent the onset of conditions associated with poor diet and a sedentary lifestyle.

Asthma, COPD, Smoking Cessation education, and management thereof

The purpose of the TLC program provides individualized case management support for patients who are willing and motivated to participate in a supportive health education program. Provides focused education for patients with hypertension and/or dyslipidemia. This program is designed to treat behaviours through lifestyle changes gained with education.
